Program Details
| The Parents And Children Together (P.A.C.T.) program is a preventative home visiting program for pregnant women living in Delta or Surrey. Family Support Workers visit cllients in their homes and offer holistic support based on individual family strengtrhs and needs. Expectant mothers are also connected to existing community services. The goal of this program is to decrease such factors as low birth weight babies, levels of isolation, and child abuse and neglect. The program also:
- Provides support and information to ensure a healthy pregnancy and birth
- Promotes positive parent-child relationships
- Promotes healthy child growth and development
- Empowers and motivates families

Eligibility Criteria
| This program is open to pregnant women living in Delta or Surrey. Referrals are accepted from pregnant women directly, nurses at the Pre-Registraion Unit at Surrey Memorial Hospital or from other professionals in the community. A screening tool is used by the Coordinator for each referred client to determine which families are eligible for the program based on the following factors:
- Age
- Limited support
- Housing concerns
- Limited education
- Lack of phone
- Limited prenatal care
- Parenting concerns
- Violence / Abuse
- Single Parenthood
- Low income / income assistance
- Tobacco / alcohol / drug use
- Isolation (social, cultural, language)
- Inadequate nutrition or weight gain during pregnancy
